BackgroundIs clomiphene citrate the same as clomiphene citrate?The current evidence supports a direct link between clomiphene citrate and the risk of heart disease. However, there is currently no definitive evidence to support an association between the use of clomiphene citrate and a higher risk of coronary heart disease. Further, the association does not support the hypothesis that clomiphene citrate is a risk factor for the development of atherosclerosis.
Clomiphene citrate is used to induce ovulation in women who have an intact uterus and who wish to become pregnant, but do not wish to become pregnant. This is because clomiphene citrate can induce ovulation by acting on the hypothalamic-pituitary-ovarian axis, which is stimulated by ovulation. Clomiphene citrate may therefore be a risk factor for coronary heart disease. The aim of the study was to examine the effect of clomiphene citrate on coronary heart disease risk in women using a method of pregnancy testing, and in a cohort of patients who had coronary heart disease. This was a prospective observational study of women who used clomiphene citrate and a control group of patients who did not use the drug. The study was conducted between the first and second trimester of pregnancy. At the end of the study, a total of 2447 women were enrolled, of whom 397 were treated with clomiphene citrate and 1135 with a control group. The primary outcome was coronary heart disease risk in patients who were treated with clomiphene citrate. The secondary outcome was the use of a control group of patients who did not use clomiphene citrate. The study was registered atClinical Research Study 2 (CRS-2)in theInternational Prospective Register. The study was stopped prematurely when the results were not published. There were 947 women in the study group and 935 in the control group. The mean age was 29.6 years in the study group and 31.5 in the control group. There was a significant difference in the rate of coronary heart disease between the study group and the control group, with a hazard ratio of 1.28 (95% confidence interval 1.08 to 1.61, p<0.001). The study group had a lower absolute risk of coronary heart disease than the control group (5.0% vs 2.4%, p<0.001). The mean age at study entry was 33.3 years in the study group and 32.2 years in the control group. The absolute risk of coronary heart disease was 6.6% in the study group and 5.7% in the control group. There was no significant difference in absolute risk of coronary heart disease between the study group and the control group (6.0% vs 3.4%, p=0.9). The absolute risk of coronary heart disease was higher in women with coronary heart disease than in women without coronary heart disease (5.0% vs 1.0%, p<0.001). The use of clomiphene citrate is associated with an increased risk of coronary heart disease in women using the drug.

The use of clomiphene citrate was associated with a higher absolute risk of coronary heart disease (5.0% vs 1.4%, p=0.002). The absolute risk of coronary heart disease was significantly higher in women using clomiphene citrate compared with the control group (3.4% vs 1.0%, p=0.006). The absolute risk of coronary heart disease was higher in women with coronary heart disease than in women without coronary heart disease (3.4% vs 1.0%, p=0.002). The use of clomiphene citrate may be associated with a higher absolute risk of coronary heart disease (5.0% vs 1.4%, p=0.001).

The risk of coronary heart disease in women using clomiphene citrate was also increased with a higher dose (50 mg) compared with a lower dose (25 mg) of clomiphene citrate (adjusted hazard ratio 1.21, 95% confidence interval 1.07 to 1.30, p=0.01).
